In entering our discussion of the arthritis known as “gout,” it is important to realize that there are severe, crippling forms of this disease. With a historical reputation of attacking only the rich, in fact gout touches all kinds of people, from all walks of life. Tne best news is that it is very controllable and represents one of medical science’s great treatment advances. Technically, gout is an accumulation in the joints of sharp crystals formed from one of the body’s natural waste products, “uric arid”/For reasons that are unknown, some people manufacture top much uric arid and others can't get the uric acid out of their body quickly enough! to avoid oversaturating the blood. Too much urip arid in. the blood behaves the way winter air saturated with water acts — it forms ice or snowflakes. In the case of uric arid saturation, sharp, rod-like crystals form which then tend to get caught in the joints. THE JOINTS most affected are the big toes, the knees, elbows, fingers and ankles. The first sympton is severe pain, although experienced gout sufferers may notice twinges in advance of a full blown attack. After a few hours the affected joint is swollen and tender. Most people can’t even endure having a sheet laid over the affected area. The skins over the joint generally looks red, shiny and dry. There is often a body temperature of 101 degrees Fahrenheit. If you think you are having a gout attack, one thing not to do is to take aspirin to ease the pain. Aspirin

slows down the excretion of uric add and can make matters worse. This is your opportunity for prompt preventive action. Bring your symptoms to your family doctor for prompt evaluation: Gout is a life-long disease; even if the symptoms disappear, (which they generally will for months or years) your body is busy laying those painful crystals down in your points, preparing for a future attack. What is most worrisome is that of “silent damage" to the kidneys. .One quarter to one fifth of those with gout can develop severe kidney problems. Kidney stones, for example, occur much more frequently in those with gout. Also, kidney diseases can lead, to gout, as they slow the rate the body can get rid of uric arid. Untreated, gout can lead to death from kidney disease, to high blood pressure and is a risk factor for stroke and heart disease. BY THE TIME you discover symptoms of gout, it is well established in your system. Prevention of further deterioration will require strict adherence to a regimen of diet, medication and medical follow-up. There are excellent medicines available to help the body speed up its elimination of uric arid, as well as medicines to keep it from producing too much uric acid in the first place. Since the process of ridding the body of uric arid is a continuous part of your life cycle, like breathing, so too must you adhere to“ the dosage and timing of your medications. Your family doctor will monitor your blood and urine over the years to make sure that your treatment remains adequate, and to ascertain if you need increased^ or decreased medication dosages. Foods lo be totally avoided because they contain a chemical which the body converts directly to udc arid include: sweetbreads, liver, kidney, brpins, sardines, anchovies and meal extracts. Drinking large quantities of fluids is important to help the tbdenys pass off the excess uric acid. Avoidance of alcoholic beverages has been found to help and, of course, if obese, returning to a health weight can reduce uric arid levels. Yes, gout can be a painful crippling disease. Untreated, joints will become deformed, kidneys may suffer damage and hypertension; stroke and heart disease are possible, treatment and prevention can tame gout’s obstacle to your good health. Health Watch is a public education project of the Cape May County Unit of the American Cancer Society, of which Dr. Beitman is co-chairman.
