Del Brooks. Social Security Manager here, said recently. Medical insurance is the part of Medicare that helps pay for doctor bills, outpatient hospital services, and other items and services not covered by hospital insurance. To receive payment under medical insurance, a person generally submits a request for payment form | and the doctor or other sup- | plier provides an itemized 1 bill to send with the form

vice provided, the date the service was provided: the place the service was provided: the charge for each service provided; the doctor or supplier who providI ed each service, and the person's name and health insurance claim number exactly as they are shown on the person's Medicare card. It is helpful if the diagnosis is also shown. For more information, ask for a free copy of the leaflet.
