"Sex and Aging" is today's topic in our continuing review of the recent New York University/ Medical School course on Geriatric Medicine. We will take this topic in two parts, this week dealing with men and next week , with women. While, this is a topic fraught with social overtones. we will discuss the physical realities and some psychological considerations. upon which each individual will super-impose his or her own value system. A sad vignette offered by my wife Gail, who is a special worker, illustrates the continuing negative view of "senator sexuality" in our society: A couple in their 70 s were admitted by their family to the Philadelphia nursing home. The home maintains separate residential floors for men and women, thus they could not rooifr together. One day a member of the nursing staff discoverd the couple together, in the wife's room, making love with the door closed. The nurse reported this to the administration, which attempted to evict both partners for breaking arule about no male/female meetings outside the community rooms. IT TOOK the threat of legal action to get the home to dismiss the idea of evicting the couple and to change their policy about interpersonal privacy. For men there are four recognized variations in their physical sexuality, all of which are normal aspects of the aging process. Although it should be noted some men will experience little change in sexual interest and ability during their lifetimes. The first variation is that starting somewhere in the mid«59's, a man may begin to take longer to achieve a full erection. When he was younger, this might have been almost automatic response to erotic stimuli like the suggestions of sex. a photograph, or seeing a sejpially attractive person on the street'. INCREASINGLY he will now need direct physical contact with hislorgan by himself and/ or his partner to achieve a full Wection. With advanced age\ the interval between enactions - may be come a matter of hours, days or months and still be considered normal. The second and third variations involve the actual nature of the man's sexual emission. The

volume of material expelled may be reduced, as well (as the pressure. These are normal changes that can occur. The fourth variation is the one which can really concern people because they don't understand it. Research has shown that in one out of each three or four sex acts, the older male will not feel the need to complete the sexual interlude bV having an orgasm. He\ill still desire and enjoy sexual activity, but simply on occasion won't feel a need to put in the extended effort to achieve orgasm THESE FOUR varia tions hold psychological, as well as physical conse-. quences. Aging men too often interpret these signs as mean that they are becoming impotent, which many men have been conditioned to believe will happen with age (Believing the'onest of impotence with aging is one way of making^ yourself impotent > Knowing the expected changes)allows a man to be himself and experience sexuality to the level possible for him and his partner Furthermore, the wife may interpret this occasional lack of need for orgasm, and slower acheivement of erection as a loss of her husband's sexual interest in her. or her inability to fulfill him In reality, he is experiencing a normal physical phenomenon. As we live longer, the clinical condition known as "Widower's Syndrome" is becoming more apparent. In men, this occurs when they have held back from sex for a year or more dur ing the illness and death of thier spouse. Sometime thereafter, they may acquire a new partner and during their first attempts at sex. discover that they can't achieve or maintain an erection. Too often the couple may give up on sexuality. In reality, the old adage "if you don't use it you'll lose it" may be coming into play. However, assuming no physical defect and that guilt over getting a new partner is not, the source of the impotence in many cases time, patience and creativity will lead to the resurgence of an enjoyable sexual union.
