This week jn our con ti nil's ing look at the services of j the Health Department's I Nursing Division, 'I am going to focus on the Case 1 Management Unit of the Department's Special Child Health Services. The Special Child Health Services Program (formerly Crippled Children's Program ) under the New Jersey State Department of Health provides furftiing through contracts with health care providers to provide quality evaluation and treatment service to all handicapped or potentially handicapped children regardless of economic status. THE SPECIAL Child Health Services Program maintains a registry of children who are at risk of developing a handicapped 1 condition. This information is- confidential and not opened to pub li^c disclosure. Hie purpose of the registry is for research and program development. The purpose of Case Management is to assure that these children receive the full range of services necessary for t^eir needs through counseling and the development of an individual service plan. This service plan identifies ~ibe children's needs and establishes objectives to assist in the best development of the child. ANY CHILD from 8 to 21 years of age with a disease, defect, or condition that interferes with normal growth and development is eligible for entry into the
tion of the child's strengths and needs. THE CASE MANAGER will also discuss the childs/family rights and responsibilities and will assist the need for services. Along with the parents, the manager will develop an individual service plan for the child. j The family will be assisted in inching the goal identified in the ser y vice plan. The case manager will take an active part in getting service through referrals to the Special Child Health Serivces network and other appropriate agencies. She will act as a resource person for the family in time of crises and promote communication among the many providers servicing the child including the child's physician. THE CASE manager also monitors the services received by the child/family by reviewing with the parent the services and the child's progress toward reaching goals identified in the individual service plan. The funding for this program is provided by the County Board of Freeholders and the State Department of Health.
