Dear Dr. Beitman: My father was admitted to the hospital because over the course of a few weeks he had become shaky, confused and could not hold his urine. After a few tests it turned out he had gallstones. How could this be? These were not the symptoms you listed in your article. We figured he was just getting old. Mark D., Wjldwood Dear Mark: It is a curous fact that often elderly persons will not exhibit the expected symptoms of an illness. I suspect an infection or disease process whenever my elderly patients who have been clear minded, able to eat and communicate, suddenly or over a short period appear feeble, confused and/or lose control over their bodily functions. SENILITY IS not a rapid process, as we have discussed in this column beforg. Individuals showing sudden confusion and/or other symptoms listed should seek prompt medical attention. A stroke might be in progress, bowel obstruction, appendicitis, an ulcer perforating the wall of the stomach, gallstones or any 6ther disease process. This is why I take particular care in getting to know my nursing home patients. The degree of mental confusion in nursing home patients spans a wide spectrum from totally clear-minded to totally senile. The doctor has to be able to judge whether the nursing home resident is simply acting normally for him/her or if a change in mental status might signal illness. IN A STUDY reported in Britain's medical journal, The Lancet, there was a report of six patients in their 80s, all of whom were seen for the sympotoms we discussed above, all of whom turned out to have gallbladder disease. They did not exhibit any of the usual features of the disease, such as pain, fever and jaundice (yellowing). Studies found an abnormal value for a liver enzyme in the blood, which led to further studies which found the gallstones. All six patients received treatments based upon the V j/ * I

extent of their disease, ranging from a course of antibiotics to surgery'. All were able to return to life outside the hospital. As we discussed two weeks ago, many cases qf gallstones are silent However, in the elderly, they may actually be speaking in a language which differs from what we tend to^xpecL TODAY I ALSO want to address two other ques tions on gallstones submitted by Health Watch readers. To the best of my knowledge lasars are not yet being used in gallbladder surgery, although they are used in some other digestive diseases and particularly in eye surgery. With regard to what type of doctor to discuss your gallstone problem with, you have several choices. Since gallstones ire such a common problem, most medical practitioners have experience with the disorder. One might ask his/her family doctor or internal medicine specialist, ■consult a digestive disease specialist or general surgeon. RECENTLY many insurancc. companies have begun suggesting second opinions before approving surgical treatment. There is a great deal otomtroversy in the medicaniterature as to when to intervene surgically in gallbladder disease, since non-surgical treatment is available in ' some cases. As a result of this, you may find a range of opinions among the doctors you consult.
