Continuing with our discussion of acute and chronic pancreatitis. All the causes are not yet fully discovered. Normally the products secreted by the pancreas pass through a duct into the small intestine. In pancreatitis, it is believed that these enzymes attack the pancreatic tissues. This damages the gland, and is sometimes accompanied by bleeding which can lead to shock, and to the formation of cysts and ab&cesses. GALLSTONES are associated with pancreatitis. About half those who go into an acute attack have gallstones. Both chronic and acute cases are often associated with chronic excess alcohol consumption. Many alcoholics either eventually die from severe pancreatitis or spend prolonged periods in the hospital receiving intravenous nutrition to overcome their destroyed pancreas' inability to process food for energy. Other causes might be certain drugs, a perforated ulcer, hyperparathyroidism , pregnancy, cancer, abdominal surgery, violent attack on the abdomen (as in an automobile accident or sports injury) and rarely, from infections such as the mumps. In 20 to 30 percent of cases the cause of the condition is unknown. AS WE discussed last week pain is the most common symptom. In the acute case it is an agonizing pain in the middle of your abdomen that may start 12 to 24 hours after a large meal or heavy bout of drinking. You may vomit, run temperature, even go into shock. This is a medical emergency and you should seek attention immediately. Chronic cases generally have a cramping pain that seems to bore into you. Sometimes it is relieved by sitting up and leaning forward. YOU MAY HAVE indigestion between attacks. You may appear yellow (jaundiced) and lose weight without trying. The pancreas may become less able to manufacture insulin and diabetes will develop. Your doctor's workup in addition to the physical exam will include laboratory studies to check if your pancreas is working properly, X-rays, possibly" a

CT scan of your pancreas to look for tumor, abscesses and enlargement, or other sophisticated X-rays. A digestive specialist like myself might conduct the special new test called ERCP which, without surgery, allows the doctor to run dye into the bile lines of the pancreas and check for stones, tumors and biopsy the area to gather much helpful information. CANCER OF the pancreas is always a consideration with these symptoms. It happens to be a particularly terrible cancer, rapid and difficult to cure unless caught in its earliest stages. If you have the symptoms outlined in this article, be sure to have them evaluated by your physician, now. It could save your life.
