Dear Mr. Beitman: I have just finished reading your Herald article on low blood count. My 94-year-old son was breaking out in a rash on his face and vomited a few times and was generally not feeling well. Also, his sister has recently had hepatitis B, so I had him take a blood test. The results were that he has anemia. I was relieved it was not hepatitis, but anemia struck me as odd for him to have because he eats so well. Not only junk food but all food. He eats some vegetables, broccoli, string beans, etc. I HAVE NOT spoken with the doctor, the results were given me over the phone by the nurse and iron vitamins were prescribed. After reading more about anemia I am concerned. D.H.. Wildwood
Dear D.H.: I think it is appropriate for you to be concerned about the finding of anemia in your young child. Several thoughts come to mind. First, perhaps it would be worthwhile to discuss with your physician whether this is a serious case of anemia, or only a very minor deviation from the normal. In any case, it may be worthwhile to repeat the blood test to see if the first hemoglobin count was correct or reflected a laboratory error, which does occur on occasion. IF THE REPEAT hemoglobin confirms the presence of anemia, then the next question to be answered is "what is the cause?" You state that your child eats very well and has a good appetite, so the nutritional B12. folic acid, and iron deficiencies would be unlikely. An evaluation looking for blood in the bowel movement or urine can be helpful in determining the source of blood loss as a cause of the anemia. Other possible causes for
anemia would be destruction of blood cells once formed, referred to as hemolysis. This occurs with congenital and acquired conditions. Sometimes prescription medications can cause this, as well. ANOTHER GROUP of problems leading to anemia would include malfunctions of the red blood cell production in the bone marrow. This group includes leukemia or cancer. Special tests might include a bone marrow examination, further blood studies for iron and'the iron binding proteins, as well as a reticulocyte bone marrow to support blood production. On occasion there may be problems with the quality of the hemoglobin and conditions such as sickle cell anemia or thalessemia may be detected. Special blood tests can help in the diagnosis of these conditions. HOPEFULLY the anemia which your son has is very minor and under your physicians eye will
soon be improved. As I have pointed out in previous columns, testing for low hemoglobin is a very helpful marker. The cause of the anemia and the actual problem producing the low blood count ideally should be identified whenever possible.
