j Continuing our series on I headaches, today we will j discuss a common type of , chronic headache about j which the public seems to ! know very little, "the ! cluster headache." [ I recently had a patient give a classic description of this kind of headache in the | office. He is a large, strong, i muscular man, a construcj tion foreman. His wife | came with him — a tiny, ! gentle woman, clearly very worried about him. "Doctor Beitman, the pain in my head is killing me when I get these headaches. It feels like a knife is cutting into my J head. I came to see you as an emergency today so you would get to see how it is I during one of these spells." I COULD SEE that the man had rivers of perspiration running down his face in the effort to talk to me through his pain. His right eye was very reddened, the tear ducts hot and full. A major artery on his forehead, known as the "temporal artery", was quite visibly standing out and pulsating. He was flushed on his right side, and holding his head there. "It's behind my eye doc. The thing is I've been getting these for the last two years, during spring and fall. Always figured that it had to do with some chemicals at work. We're busiest in spring and fall." HE TELLS ME the attacks usually occur at night and last for two or three hours. I can see the attack is lessening as he talks. As part of his history and
physical I learn he was out with the boys having a few beers last night, which is more common during the busy season than the rest of the year. By now I am convinced he is suffering from cluster headaches. We order a skull X-ray and CT scan to rule out any chance of a brain tumor, and a brain study (EEG) to check for small seizures. THE CAUSES of cluster headaches have not been proven. Statistically they are most often brought on by consumption of alcoholic beverages. Nitroglycerin and histamine are also known causes. Biologically, a malfunction of the body system which regulates cycles in the nervous sytem is believed to take place due to a change in levels of certain chemicals that are effected by alcohol and the drugs mentioned. Depending upon the frequency and length of the headaches and the person's general health we can offer treatments to try to prevent the attacks, or to stop an attach which has begun. I believe headaches should be taken very seriously, the person's suffering is very real and may extend to affect family life as well. Dr. Beitman is president of the Cape May County Unit of the American Cancer Society of, which Health Watch is a public education project.
